Patient

Female, mid-40s

Presentation

Upper left first molar with persistent discomfort despite previous root canal treatment elsewhere. The referral X-ray suggested a possible missed canal.

The Challenge

Upper molars frequently harbour a fourth canal (the MB2, or second mesiobuccal canal). It is present in approximately 60% of upper first molars but is often missed during initial treatment because it is extremely narrow and hidden.

Treatment

Under microscope magnification, the MB2 canal was located as a completely separate canal (not joining the MB1 canal). All four canals were re-cleaned, reshaped, and re-obturated to ensure complete disinfection.

Outcome

Symptoms resolved fully within one week, confirming the missed MB2 was the source of ongoing infection.
